Terms of Agreement/Consent
I hereby give approval for the participation of myself in any and all GetShook activities and assume all risks and hazardous incidents to such participation, including the transportation to and from all activities. I waive, release, adsolve, indemnify and agree to hold harmless the Getshook, affiliated associations, oganizers, officers, coaches, parents, participants and officials from any claim arising out of injury to myself. Participants will receive a 50% refund if requested prior to the event. No refund will be issued due to inability or unwillingness to attend/participate. Prorated refunds will only be considered for players when a medical/physical injury/illness, verified by a physician's written statement, precludes participation or event. We understand that NO REFUNDS will be issued other than stated above. GetShook, LLC, GetShooksports, Getshook.net and GetShook and Get Shook Sports has my permission to use any pictures taken of me for the purpose of promoting their business including brochures/website.
